HERES WHAT TO DO: 1) Login to the Patient Login section and you will be taken to the HOME screen where you will see a MY TO DO LIST. 2) Please click on each link under the heading MESSAGE and then read and complete each form we have initially placed in your MY TO DO LIST. 3) Once you have read the required form scroll down to the bottom of the page and enter your password and click SIGN AND COMPLETE DOCUMENT.

PASSWORD PROTECTION INFO: The information you submit is secured and no one else has access to your clinical information as it is password protected. Once logged in, you will have the option of changing your password by clicking on change password on the left top portion of the screen. In order to get back to the main page to continue with the To Do List, click the back button at the bottom of each form. FOR PATIENTS UNDER 18: a parent or guardian will need to fill out each form for the client and electronically sign each form.
